X403 – CT Neck - without IV contrast

Computed tomography (CT) of the neck, without intravenous contrast. This diagnostic imaging procedure is used to visualize the structures of the neck. It is mutually exclusive with X404 (with contrast) and X124 (with and without contrast) for the same imaging session.

When to Use

Use X403 when performing a non-contrast CT neck for screening or baseline anatomical assessment where contrast is contraindicated or unnecessary.
Use X403 for initial evaluation of neck trauma or suspected foreign body localization where contrast enhancement would not improve diagnostic yield.

Common Pitfalls

Billing X403 in conjunction with X404 or X124 for the same patient encounter will trigger an automatic rejection due to mutual exclusivity.
Attempting to bill X403 for a study that included contrast administration is a common audit trigger; ensure the imaging protocol matches the code exactly.

If providing urgent interpretation via PACS for an acute care patient, ensure you append the appropriate E-series premium (E406, E407, or E408) to maximize the claim value.
