This service is for a repeat Magnetic Resonance Imaging (MRI) of the pelvis, performed in a different plane or with a different pulse sequence during the same examination as a primary pelvic MRI (X461). This allows for more detailed or specific views of the pelvic region. It can be billed a maximum of three times per examination. This service is a diagnostic imaging procedure and includes all common elements of an insured service as defined in the Schedule of Benefits (, ).
When to Use
- Use X465 when you perform additional sequences, such as T2-weighted or diffusion-weighted imaging, beyond the primary pelvic MRI (X461) to improve diagnostic clarity.
- Apply this code when imaging the pelvis in a secondary plane (e.g., sagittal or coronal) following the initial axial view captured under X461.
Common Pitfalls
- Billing X465 more than three times per examination will result in automatic rejection by the MOH.
- Attempting to bill X465 without a corresponding primary X461 claim on the same date for the same patient will lead to a claim mismatch.

- Ensure your documentation clearly specifies the additional pulse sequence or plane used to justify each unit of X465 claimed.
