Magnetic Resonance Imaging (MRI) of a limited spine (one segment), representing a repeat scan performed in another plane or with a different pulse sequence. This service follows an initial sequence (X490) and is subject to a maximum of 3 repeats.
When to Use
- Use X492 when performing a secondary pulse sequence or plane change for a limited spine MRI immediately following the primary scan billed as X490.
- Use this code for up to three distinct repeat sequences per patient encounter to capture additional diagnostic planes not covered by the initial X490 fee.
Common Pitfalls
- Billing X492 without a corresponding X490 on the same claim will result in an automatic rejection as it is strictly a secondary service.
- Exceeding the maximum of three units of X492 per patient encounter will trigger a rejection, as the Schedule of Benefits limits repeat sequences for limited spine MRIs.
