Abscess or haematoma - general anaesthetic - perianal (Integumentary System, Incision). E515 adds 110% when performed as the sole procedure under general anaesthetic in an operating room, but not in an emergency department or equivalent.
The medical record must establish that: 1. an insured service was provided; 2. the service for which the account is submitted is the service that was rendered; and 3. the service was medically necessary.
