Z146 – Two lesions
OHIP Psychiatric Code — INTEGUMENTARY SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
Surgical procedure for the excision of two lesions on the face or neck under general anaesthetic, for conditions such as cyst, haemangioma, or lipoma. As a surgical service, it includes pre-operative evaluation, the procedure itself, and post-operative care as defined in the surgical preamble. Payment for surgeon, assistant, and anaesthetist services is based on fee values and may incorporate time units.
When to Use
- Use Z146 when performing the excision of exactly two distinct lesions located on the face or neck under general anaesthetic.
- Use this code for the simultaneous removal of two benign growths, such as two lipomas or two cysts, when the surgical site is limited to the face or neck region.
Common Pitfalls
- Do not bill Z146 in conjunction with individual lesion excision codes like Z145; Z146 is a bundled code specifically for the two-lesion count.
- Avoid billing Z146 if the lesions are located outside the face or neck, as this code is anatomically restricted to those regions.
- Ensure the procedure is performed under general anaesthetic; if the procedure is performed under local anaesthetic, Z146 is not the correct billing code.
Billing Tips
- If you remove more than two lesions, you must use the appropriate surgical codes for the additional lesions, as Z146 is strictly defined for a count of two.
- Always document the specific anatomical location of both lesions on the face or neck in the operative report to support the use of this specific code during an audit.
