Z265
Z265 – Costochondral or chondral graft - one rib
OHIP Psychiatric Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
Surgical harvesting of a costochondral or chondral graft from one rib. This service is intended for when the harvesting of a graft is not an intrinsic component of the primary surgical procedure being performed. For harvesting from additional ribs during the same operation, see E976.
When to Use
- Use Z265 when harvesting a costochondral or chondral graft from a single rib as a distinct procedure separate from the primary surgical site reconstruction.
- Apply this code when the graft harvest is not considered an intrinsic or mandatory component of the primary procedure being performed.
Common Pitfalls
- Billing Z265 when the graft harvest is already included in the fee for the primary surgical procedure, which constitutes unbundling.
- Failing to use E976 for additional ribs harvested during the same session, as Z265 only covers the first rib.
Billing Tips
- Always verify if the primary procedure's description includes graft harvesting; if it does, Z265 is not payable as a separate service.
- Ensure the age premium is applied correctly for patients under 16, as this is a surgical procedure in the Z-section.
