A procedure for obtaining a tissue sample from the lung using a needle. As this is an Independent Operative Procedure (IOP) identified by the 'Z' prefix, it is subject to specific pre- and post-operative billing rules outlined in the Surgical Preamble. See for details.
When to Use
- Use Z340 for percutaneous needle biopsy of the lung when performed as a standalone procedure.
- Use Z340 when obtaining a tissue sample via needle aspiration or core biopsy, provided the procedure does not escalate to a more complex surgical intervention.
Common Pitfalls
- Billing Z340 in addition to a consultation on the same day; as an IOP, the pre-operative assessment is generally included in the procedural fee.
- Failing to apply the 85% rule when Z340 is performed alongside another surgical procedure during the same operative session, as the major procedure takes 100% and subsequent procedures are reduced.
- Attempting to bill Z340 when the biopsy is performed as part of a more extensive thoracic surgery, where the biopsy should be considered incidental to the primary procedure.
Billing Tips
- If Z340 is performed bilaterally or as multiple IOPs in one session, claim the full procedural fee for each, but ensure the pre- and post-operative care is only claimed once as per the Surgical Preamble.
- Always document the specific needle technique and the anatomical site to support the medical necessity of the Z340 code during potential Ministry audits.
