Z345 – Subsequent procedures performed by same physician

This fee code applies to a subsequent tracheo-bronchial aspiration procedure performed by the same physician who rendered the initial aspiration (Z344). This is classified as an Independent Operative Procedure (IOP) and is subject to the billing rules outlined on page of the Schedule of Benefits. Specifically, it is not payable for: - operating surgeons - when respiratory unit fees apply - within the first two hours post-operatively.
When to Use
- Use Z345 for a second or subsequent tracheo-bronchial aspiration performed by the same physician after the initial Z344 has been completed.
- Use this code when the patient requires repeat suctioning due to persistent secretions, provided the procedure occurs at least two hours after the initial Z344.
Common Pitfalls
- Billing Z345 within two hours of the initial Z344 will result in an automatic rejection as the code is explicitly prohibited during the immediate post-operative window.
- Attempting to bill Z345 alongside respiratory unit fees is a common error; these are mutually exclusive and the respiratory unit fee takes precedence.
- Operating surgeons cannot bill Z345, as the aspiration is considered inclusive of the primary surgical procedure's global fee.
Billing Tips
- Ensure the time of the procedure is clearly documented in the chart to justify the two-hour interval required between Z344 and Z345.
- If performing the procedure after hours, remember that Z345 is eligible for E409 or E410 premiums, provided the procedure is non-elective.
