Z349 – Intrapleural administration of chemotherapy or sclerosing agent

This is an Independent Operative Procedure (IOP) for the intrapleural administration of chemotherapy or a sclerosing agent. This procedure can be performed by any method and is used in the treatment of conditions related to the lungs and pleura. As a surgical service, it includes the necessary pre-operative and post-operative care as outlined in the Surgical Preamble (:529).
When to Use
- Use Z349 for the instillation of a sclerosing agent, such as talc or doxycycline, into the pleural space to manage recurrent malignant pleural effusions.
- Use Z349 for the intrapleural administration of chemotherapy agents when performed as a distinct surgical procedure.
Common Pitfalls
- Billing Z349 alongside a major assessment on the same day by the same physician will result in the assessment being rejected, as Z349 is an Independent Operative Procedure (IOP).
- Attempting to bill Z349 in addition to a consultation on the same day is prohibited; the consultation will be denied if the IOP is performed by the same physician.
Billing Tips
- If the procedure is performed after hours, ensure you append the appropriate premium code (E409 or E410) to the Z349 claim to increase the procedural fee by 50% or 75% respectively.
- When performing Z349 in conjunction with other surgical procedures during the same hospitalization, remember that the 15% reduction rule applies to the lesser procedure(s) per the Surgical Preamble.
