Z358 – Thoracic window closure
OHIP Psychiatric Code — RESPIRATORY SURGICAL PROCEDURES · Schedule of Benefits
A surgical procedure for the closure of a thoracic window. As an Independent Operative Procedure (IOP), the fee includes the major pre-operative visit and post-operative care associated with the procedure. The specific elements of this surgical service, as outlined in the Surgical Preamble (), include supervising and/or preparing the patient, performing the procedure, arranging for related assessments or therapy, post-procedure monitoring until the first post-operative visit, and providing advice and information to the patient or their representative.
When to Use
- Use Z358 for the definitive surgical closure of a thoracic window where the procedure is performed as an Independent Operative Procedure (IOP).
- Select this code when the clinical requirement is specifically the closure of a thoracic window, distinguishing it from general thoracic debridement or other respiratory surgical procedures.
Common Pitfalls
- Billing an office consultation or assessment on the same day as Z358 is a common rejection, as the IOP fee includes the major pre-operative visit.
- Failing to apply the 85% payment rule for secondary procedures when Z358 is performed alongside other surgical interventions during the same operative session.
- Attempting to bill separate post-operative visits within the global period, which are already included in the Z358 procedural fee.
Billing Tips
- Ensure the surgical report clearly documents the closure technique to support the use of Z358 over less specific respiratory procedure codes.
- If Z358 is performed after hours, ensure the claim includes the appropriate E-series premium (E409 or E410) to increase the procedural fee, provided the criteria for non-elective status are met.
