- Use Z400 when performing an urgent EGD specifically to identify and manage active upper gastrointestinal hemorrhage.
- Select Z400 over Z399 when the primary clinical indication is the active bleeding state, as Z400 is the designated surgical code for this acute pathology.
Common Pitfalls
- Billing Z292 (laryngoscopy) alongside Z400 will trigger an automatic rejection unless you provide a manual written explanation justifying the suspicion of laryngeal disease.
- Attempting to claim a surgical assistant fee for Z400 will result in a rejection, as the assistant value for this code is set to 'nil' in the Schedule of Benefits.
Billing Tips
- Always append the appropriate Special Visit Premium (Table I, II, or III) if the procedure is performed on an emergency basis, as Z400 does not include the premium in its base fee.
