Z411 – Axillary or inguinal lymph node(s), unilateral

This service is for the excision of one or more axillary or inguinal lymph nodes on a single side (unilateral) for diagnostic purposes (biopsy). As a surgical procedure, it includes the associated pre-operative and post-operative care as defined in the Schedule of Benefits. Assistant and anaesthetist services are separately billable based on basic and time units.
When to Use
- Use Z411 for the excision of a single or multiple lymph nodes from one axillary or inguinal basin for diagnostic biopsy purposes.
- Use Z411 when performing a sentinel lymph node biopsy if the procedure is limited to a single basin and does not meet the criteria for a more extensive lymphadenectomy (e.g., Z405 or Z406).
Common Pitfalls
- Billing Z411 bilaterally is incorrect; if nodes are excised from both axillary or inguinal regions, you must bill Z411 for one side and use the appropriate bilateral modifier or secondary code as per the Schedule of Benefits.
- Do not bill Z411 in addition to a major lymphadenectomy (Z405 or Z406) on the same side, as the biopsy is considered an inclusive component of the more extensive surgical procedure.
