This service covers cardioversion, either electrical or chemical, for treating cardiac arrhythmias. It is payable for a maximum of three sessions per patient per day. When anaesthesia is required, it is billed separately using the 'C' suffix, with 6 base units allocated for the anaesthesiologist's service. As a Z-code procedure, it may be eligible for various premiums such as those for after-hours service, age, or trauma, subject to specific criteria.
When to Use
- Use Z437 for the delivery of a synchronized electrical shock to terminate atrial fibrillation or flutter in a hospital setting.
- Use Z437 when administering intravenous antiarrhythmic agents specifically for the purpose of chemical cardioversion of an acute arrhythmia.
- Use Z437 for repeat cardioversion attempts within the same day, up to the maximum of three, if the initial rhythm conversion fails.
Common Pitfalls
- Billing Z437 in conjunction with a consultation or assessment code (e.g., A007) for the same patient on the same day often triggers a rejection unless the assessment is for a separate, unrelated clinical issue.
- Failing to bill the anaesthesia component as Z437C when sedation is provided by a second physician, as Z437 only covers the procedure itself.
- Attempting to bill Z437 for routine rhythm monitoring or ECG interpretation, which are included in the global fee of an assessment or are covered by separate diagnostic codes.
Billing Tips
- Always append the appropriate after-hours premium code (e.g., E409) if the procedure is performed outside of standard hours, as Z437 is eligible for these procedure-based premiums.
- Ensure the clinical note clearly differentiates the cardioversion procedure from any preceding assessment to justify billing both if the patient presents with a new, distinct problem.
