A surgical procedure involving one or more biopsies of the vulva and/or introitus, performed under local anaesthetic. This service is only eligible for payment when it is the sole procedure performed during the encounter. An add-on premium, E542, is payable when this service is rendered in a non-hospital setting.
When to Use
- Use Z477 for a standalone vulvar or introital biopsy performed under local anesthetic when no other surgical procedures are performed during the same encounter.
- Select Z477 instead of Z475 when the biopsy is the only procedure, as Z475 is intended for biopsies performed in conjunction with other surgical procedures.
Common Pitfalls
- Billing Z477 alongside any other surgical procedure code will result in a rejection, as it is strictly defined as a sole procedure code.
- Attempting to bill Z477 for biopsies performed under general anesthesia or sedation is incorrect, as the code specifically requires local anesthetic.

- Always append E542 to your claim if the procedure is performed in a non-hospital setting to capture the eligible premium.
