When to Use
- Use Z705 for a formal bilateral testicular biopsy performed in a surgical setting, distinct from a simple needle aspiration.
- Use Z705 when the procedure is performed as a primary surgical intervention, rather than as a component of a more extensive procedure like an orchiectomy or radical exploration.
Common Pitfalls
- Billing Z705 alongside an admission assessment (A933/C933/C003/C004) is prohibited if the surgeon performed a pre-operative assessment within 30 days of the admission.
- Attempting to bill Z705 in addition to a consultation fee for the same patient on the same day is generally rejected unless the consultation meets the criteria for a 'major pre-operative visit' where the decision to operate was finalized.
Billing Tips
- Ensure the procedure is clearly documented as bilateral to justify the Z705 fee, as unilateral biopsies may fall under different coding logic or require manual review.
- If the procedure is performed on an emergency basis after hours, ensure the appropriate E-series premium (E409 or E410) is applied to the procedural fee to maximize reimbursement.
