Open surgical insertion of a circulatory assist device, e.g., an intra-aortic balloon. The fee for this procedure is inclusive of cannulation, repair of the artery, as well as daily care and supervision of the device as noted on page of the Schedule.
When to Use
- Use Z743 for the open surgical insertion of an intra-aortic balloon pump (IABP) when the procedure involves direct arterial access and repair.
- Select Z743 instead of percutaneous codes when the clinical situation requires an open cut-down approach for device placement.
Common Pitfalls
- Do not bill for daily management or supervision of the device separately, as these services are explicitly included in the Z743 fee.
- Avoid billing for the initial cannulation or subsequent arterial repair as separate procedures, as these are bundled into the Z743 payment.
- Do not attempt to claim Z743 alongside Z780 for the same device insertion, as this constitutes unbundling of the surgical service.
Billing Tips
- When performing a graft of a major vessel for a ventricular assist device in conjunction with Z743, ensure you append the E682 add-on code to capture the additional surgical complexity.
