- Select Z769 for extensive or recurrent condylomata cases that cannot be managed in an office setting under local anaesthetic, which would otherwise be billed under Z733 or Z736.

All insured services must be documented in appropriate records, establishing that the service was provided, is the service submitted, and was medically necessary.
For anaesthesia services, the level determined by the anaesthesiologist at the time of the pre-operative anaesthesia assessment must be recorded in the medical record if claiming extra units for ASA physical status.
If claiming the trauma premium (E420), the medical record must list the Injury Severity Score (ISS).
