Endometrial sampling is a procedure to obtain a tissue sample from the uterine lining. An additional fee (E542) is payable when this procedure is performed outside of a hospital. A paracervical block (E861) is also payable in addition when performed by the same physician in an office.

When to Use

  • Use Z770 for office-based endometrial biopsies performed for abnormal uterine bleeding or post-menopausal bleeding investigations.
  • Use Z770 when performing a diagnostic sampling to rule out endometrial hyperplasia or malignancy in an office setting.

Common Pitfalls

  • Failing to bill E542 when performing the procedure in a community office, as this premium is not automatically triggered by the location code.
  • Attempting to bill a consultation or assessment code (e.g., A007) on the same day as Z770 without a separate, distinct clinical reason and appropriate modifier, which often leads to rejection.
  • Billing E861 for a paracervical block when the procedure is performed in a hospital setting, as E861 is strictly restricted to office-based procedures.

Billing Tips

  • Always append E542 to your Z770 claim for office procedures to ensure you receive the full procedural compensation.
  • If you perform a paracervical block to facilitate the biopsy in your office, ensure both Z770 and E861 are submitted on the same claim to capture the full allowable fee.
