- Failing to bill E542 when performing the procedure in a community office, as this premium is not automatically triggered by the location code.
- Attempting to bill a consultation or assessment code (e.g., A007) on the same day as Z770 without a separate, distinct clinical reason and appropriate modifier, which often leads to rejection.
- Billing E861 for a paracervical block when the procedure is performed in a hospital setting, as E861 is strictly restricted to office-based procedures.
