Removal of one foreign body from the cornea, under local anaesthetic. This service is a surgical procedure listed in the Ocular and Aural Surgical Procedures section of the Schedule. For the removal of two foreign bodies, see Z848. For three or more foreign bodies, see Z845. For removal requiring a general anaesthetic, see Z852.

When to Use

  • Use Z847 for the removal of a single corneal foreign body requiring local anaesthetic, such as a metal filing or glass shard.
  • Use Z847 when the procedure is performed in an office or clinic setting under topical or local infiltration anaesthesia.

Common Pitfalls

  • Billing Z847 multiple times for the same eye on the same day is a rejection risk; use Z848 for two bodies or Z845 for three or more.
  • Do not bill Z847 if the procedure requires general anaesthesia, as this must be billed under Z852.
  • Attempting to bill a separate office visit fee (e.g., A007) alongside Z847 is often disallowed unless the visit is for a separate, unrelated clinical issue.

Billing Tips

  • Ensure the number of foreign bodies is clearly documented in the chart to justify the selection of Z847 over Z848 or Z845.
  • If the procedure is performed in an emergency department setting, ensure you evaluate eligibility for Special Visit Premiums (Table I) in addition to the base Z847 fee.
