When to Use
- Use Z874 for the surgical incision and curettage of a chalazion performed under local anesthesia.
- Use Z874 when managing multiple chalazia in the same eye or different eyes during a single session, as the code covers single or multiple lesions.
Common Pitfalls
- Billing Z874 alongside a minor assessment code (A007) is generally disallowed as the assessment is considered included in the surgical fee.
- Mistaking a hordeolum (stye) for a chalazion; Z874 is specific to chalazion excision and may be audited if the clinical diagnosis does not support the procedure.
- Failing to append E542 when the procedure is performed in a physician's office rather than a hospital setting, resulting in lost revenue.
