A011 – Focused Practice Repeat Consultation by Video

A repeat consultation rendered by a GP Focused Practice Physician who completes a full, relevant history of the presenting complaint and detailed examination of the affected part(s), region(s), or system(s) needed to make a diagnosis, and/or exclude disease, and/or assess function, provided via a verified video platform.
When to Use
- Bill A011 when a GP Focused Practice Physician provides a repeat video consultation for a patient whose condition has been managed by another physician since the initial A010 consultation.
- Use A011 for a follow-up video consultation on the same presenting complaint, provided there has been intervening care from another physician, and the patient is in Ontario.
- A011 is appropriate for a repeat video consultation where the physician performs a detailed examination of the affected part(s) via video, following interval care by another provider.
Common Pitfalls
- Billing A011 without documentation of intervening care by another physician since the initial A010 consultation will lead to claim rejection.
- Submitting A011 when the patient is not physically located in Ontario at the time of the video consultation is a common error.
- Using A011 when the patient has not received care from another physician since the initial consultation (A010) is incorrect; this scenario may require A010 again if criteria are met, or another appropriate code.
Billing Tips
- Ensure the referral request for the A011 consultation is kept on file, clearly identifying the referring physician, patient, and the service required.
- Remember that A011 is exempt from the 'one per 24 months' limit for standard consultations, provided the repeat consultation criteria (interval care) are met.

Consultation
A written report (including findings, opinions, and recommendations) to the referring physician, nurse practitioner or dental surgeon must be prepared.
A copy of the written request for the consultation, signed by the referring physician, nurse practitioner or dental surgeon must be kept in the consulting physician’s medical record, except in the case of a consultation which occurs in a hospital, long-term care institution or multi-specialty clinic where common medical records are maintained. In such cases, the written request may be contained on the common medical record.
The request identifies the consultant by name and/or the specialty being consulted, the referring physician, nurse practitioner or dental surgeon by name and billing number, and identifies the patient by name and health number.
The written request sets out the information relevant to the referral and specifies the service(s) required.
Must be rendered following care by another physician in the interval since the initial consultation.
Must use a Verified-Virtual Visit Solution.
Both physician and patient must be in Ontario.
Classified as a Limited Virtual Care Service (rendered outside an existing/ongoing relationship).
The fee is equivalent to the in-person repeat consultation fee (A006).
