Consultative interview on behalf of disturbed patient (including report) - consultative interview with patient less than age 22
Requires a written request from a referring physician, nurse practitioner, or dental surgeon.
A written report (including findings, opinions, and recommendations) must be prepared and sent to the referring provider.
The physician must spend a minimum of 60 minutes in direct contact with the patient.
The service must be rendered personally by the attending physician.

Usage limits
Consultations are generally limited to one service per two consecutive 12-month periods for the same patient by the same physician for the same diagnosis, unless it is a repeat consultation or meets specific hospital/ED criteria.
If A197 and A198 are rendered to the same patient on the same day, the minimum combined time required before the first unit of K630 is payable is 120 minutes (total 136 minutes for consultation + 1 unit of K630).
Calculation of time excludes time devoted to other separately billable interventions and non-patient-facing time (e.g., chart review).
