A medical specific re-assessment is a service rendered by a specialist in Occupational Medicine (72) that requires a full, relevant history and physical examination of one or more systems.
When to Use
- Bill A724 for a follow-up occupational medicine assessment of a patient experiencing a new symptom related to a previously diagnosed work-related injury, requiring a focused history and physical exam of the affected system.
- Use A724 when a patient requires a re-evaluation of their work capacity after a change in their medical condition, and the assessment involves a detailed review of their occupational exposures and a physical examination.
- A724 is appropriate for a specialist in Occupational Medicine to assess a patient who has had a prior occupational medicine assessment (e.g., A720 or A723) and now requires a specific re-assessment of a new or worsening work-related condition.
Common Pitfalls
- Billing A724 more than twice in a 12-month period for the same patient, unless the visits are for hospital admissions, will result in the subsequent claims being adjusted to a lesser assessment fee (A728).
- Claims for A724 may be rejected if the documentation does not clearly detail a full, relevant history and a physical examination of one or more systems, as required by the code description.
- Using A724 for a routine follow-up that does not involve a new history and physical examination, or for a condition already addressed in a previous assessment, can lead to claim rejections or audits.

Admission assessments are deemed to be a specific re-assessment or medical specific re-assessment under either of the following circumstances: 1. for those procedures prefixed with a “Z” or noted as an IOP, by a surgical specialist who has assessed the patient prior to admission in respect of the same illness; or 2. for those patients who have been assessed by a physician and subsequently admitted to the hospital for the same illness by the same physician.
