A partial assessment is the limited service that constitutes a history of the presenting complaint, the necessary physical examination, advice to the patient and appropriate record.
When to Use
- Use A728 for a focused occupational medicine assessment of a new work-related injury or illness, distinct from a comprehensive consultation (A720).
- Bill A728 when providing a limited occupational medicine assessment for a patient requiring a specific work status update or medical opinion, not covered by a general assessment (A003).
- A728 is appropriate for follow-up occupational medicine visits where a new complaint requires a focused history, exam, and advice, differentiating it from a simple follow-up visit.
Common Pitfalls
- Billing A728 on the same day as a consultation (A725, A720, A925, A726) will result in rejection of the A728 claim.
- Submitting A728 for a service that constitutes a comprehensive occupational medicine consultation (A720) may lead to audit and clawback.
- Failure to document the specific presenting complaint, examination findings, and advice provided will not meet the requirements for A728.
Billing Tips
- A728 can be billed for virtual care using the 'A' suffix (A728A) for both video and telephone services.
- A728 is eligible for travel premiums (K960-K964) and first/additional person seen premiums (K990-K999) under specific circumstances.
