C008 – Concurrent care

A routine assessment rendered in hospital by a consultant following the consultant’s first major assessment of the patient when the family physician remains the most responsible physician (MRP) but the latter requests continued directive care by the consultant.
When to Use
- Use C008 when you are acting as a consultant for a specific medical issue while the patient's primary physician remains the MRP and has explicitly requested your ongoing directive management.
- Use C008 for routine daily rounds when you are providing active, directive treatment for a patient's condition that falls outside the scope of the MRP's primary management plan.
Common Pitfalls
- Billing C008 when the service is actually Supportive Care (C010); C008 requires active directive management, whereas C010 is for liaison or reassurance only.
- Exceeding the weekly frequency limits of 4 claims in the first week and 2 claims thereafter, which will trigger automatic rejections.
- Attempting to append premiums like E083 or E082 to C008, as these are restricted to MRP services and are incompatible with consultant concurrent care codes.
Billing Tips
- Ensure the medical record clearly documents the MRP's request for your continued directive care to justify the use of C008 over a standard subsequent visit code.
- If the patient develops an acute, unrelated intercurrent illness requiring additional visits beyond the weekly C008 limits, bill the additional visits using C121 rather than attempting to force them into C008.
