C312 – Subsequent visit - first five weeks

A subsequent visit is any routine assessment of a non-emergency hospital in-patient following the hospital admission assessment, rendered by a specialist in Physical Medicine & Rehabilitation (31). This service is applicable for visits rendered within the first five weeks of the patient's admission. As per the of the Schedule, a subsequent visit includes any routine assessment in hospital following the hospital admission assessment. It may also include attendance at surgery if requested by the patient, and care from multiple disciplines if required.
When to Use
- Use C312 for routine daily follow-up assessments of an inpatient by a Physiatrist within the first 35 days of admission.
- Use C312 when providing routine non-emergency care for a patient previously admitted under your care or a colleague's care, provided the 35-day window has not elapsed.
Common Pitfalls
- Billing C312 after the 35-day (five-week) threshold; you must switch to the appropriate chronic or long-stay subsequent visit code (e.g., C317) thereafter.
- Attempting to bill C312 on the same day as a consultation (A312) or an admission assessment (C311), which will result in a rejection for duplicate service.
- Failing to append the MRP premium (E083 or E084) when you are the Most Responsible Physician, as this is a missed opportunity to increase the base fee by 30% or 45%.
Billing Tips
- Always verify the patient's admission date to ensure you are within the 35-day window, as OHIP systems automatically reject C312 claims submitted beyond this timeframe.
- If the patient is in an ICU or CCU, remember to add C101 to your C312 claim to capture the $9.10 premium, provided you are not already claiming a team-based fee that excludes it.
