This service applies to routine assessments following the hospital admission assessment, specifically during the period from the sixth to the thirteenth week of admission, inclusive. When to Use
- Use C317 for routine follow-up assessments of an in-patient between day 36 and day 91 of their hospital stay.
- Use this code when the patient has transitioned past the initial 5-week period covered by C316 but has not yet reached the long-term stay threshold of 13 weeks.
Common Pitfalls
- Billing more than 3 visits per week will result in automatic rejection, as the system strictly enforces the weekly limit for this specific time window.
