  • Attempting to bill E062 in isolation; it is strictly an add-on code and will be rejected if not submitted with the base procedure R401.
  • Billing E551, E552, or E553 on the same day as E062, which triggers an automatic rejection due to the explicit restriction in the Schedule of Benefits.
  • Failing to recognize that the assistant's fee for E062 is already captured within the total time units of the primary R401 procedure.
