Add-on to a periodic health visit (K131 or K132) when rendered to a cisgender female or a transgender, nonbinary, gender fluid or agender patient. E089 is only eligible for payment when the patient's gender identity is documented in the patient's medical record.
The patient’s gender identity must be documented in the patient’s medical record.
Usage limits
Limited to one per patient per 12 month period per physician (matching the limit of the base periodic health visit codes K131 and K132).

E089 is not listed as an eligible virtual care service in Appendix J, meaning it must be rendered in person.

E089 adds 20% of the K131 or K132 fee (Schedule page ). It is submitted at $0.01 and OHIP pays the percentage.
