E441 is an add-on code to be claimed with a percutaneous epidural injection (G246, G117, G119, or G918). This premium is applicable when the injection is administered at the same vertebral level where the patient has undergone previous spinal surgery.

When to Use

  • Use E441 when performing a therapeutic epidural injection (G246, G117, G119, or G918) specifically at the site of a prior laminectomy or discectomy.
  • Apply this code when the patient's surgical history is confirmed at the exact vertebral level being injected to account for the increased technical difficulty of post-surgical scarring.

Common Pitfalls

  • Billing E441 without one of the four mandatory parent codes (G246, G117, G119, or G918) will result in an automatic rejection.
  • Claiming E441 for injections at levels adjacent to, but not including, the previously operated vertebral level is an audit risk and considered an invalid use of the premium.
