Superior labral anterior posterior (SLAP) repair is an add-on procedure payable in addition to `R684` (Shoulder arthroscopy setup). Payment Rules: E484 is not eligible for payment in addition to `R401. **Note:** Arthroscopic E-add-on codes are not eligible for payment in addition to R684` when the service described by the E-code is a generally accepted component of another shoulder procedure performed arthroscopically (see ).

When to Use

  • Use E484 as an add-on to R684 when performing a specific arthroscopic repair of a superior labral tear (SLAP lesion).
  • Use this code when the SLAP repair is the primary surgical objective performed in conjunction with the standard arthroscopic shoulder setup (R684).

Common Pitfalls

  • Billing E484 in addition to R401 will result in an automatic rejection as they are mutually exclusive.
  • Attempting to bill E484 when the SLAP repair is considered a standard component of another primary shoulder procedure will lead to claim denial under N23 guidelines.
