This is a percentage-based surgical add-on for immediate superficial chest wall reconstruction following a simple mastectomy (R108). It is intended to compensate for the additional work of extensive undermining or specific closure techniques to manage flap deformities.
When to Use
- Use E517 when performing a simple mastectomy (R108) where extensive undermining exceeding 10cm is required to manage inferior or lateral flap excess.
- Apply this code when utilizing a modified M-plasty or fishtail closure technique specifically to address skin redundancy during a simple mastectomy.
Common Pitfalls
- Billing E517 with R117 (skin-sparing mastectomy) will result in a rejection, as this add-on is strictly restricted to R108.
- Failure to explicitly document the measurement of undermining or the specific closure technique (e.g., fishtail) in the operative report will lead to recovery during an audit.
Billing Tips
- Ensure the claim includes both R108 and E517 on the same submission to trigger the 30% fee calculation automatically.

Surgery
The surgeon must perform and document one of the following in the operative report:
a. Extensive undermining for local advancement flaps >10cm to obliterate inferior or lateral excess flap deformities associated with simple mastectomies; or
b. Modified M-plasty or fishtail closure.
