E675 – Gastroscopy - with snare polypectomy - additional polyp > 1 cm

This service is for the snare polypectomy of each additional polyp greater than 1 cm. It is an add-on to a primary gastroscopy procedure and is payable with Z527, Z547, or Z528. This code can be billed for a maximum of two additional polyps, following the removal of the first large polyp which is billed under E674.
When to Use
- Use E675 for the second and third snare polypectomy of polyps > 1 cm, provided the first polyp > 1 cm was already billed using E674.
- Apply this code when performing multiple snare polypectomies during a single gastroscopy session (Z527, Z547, or Z528) where each additional lesion meets the size criteria.
Common Pitfalls
- Billing E675 without first billing E674 will result in a rejection, as E675 is strictly an add-on for additional lesions beyond the first.
- Exceeding the maximum of two units per day; E675 is capped at two additional polyps, meaning you cannot bill more than three total snare polypectomies > 1 cm (one E674 + two E675) per procedure.
- Attempting to use E675 for polyps 1 cm or smaller, which should be billed under different codes or are considered included in the base procedure fee.

- Ensure your operative report explicitly documents the size of each polyp removed to justify the use of E674 followed by E675 units.
