E682Graft of major vessel other than ascending aorta for cardiopulmonary bypass or ventricular assist device

OHIP Surgical Assists Code — CARDIOVASCULAR SURGICAL PROCEDURES · Schedule of Benefits

Graft of major vessel other than ascending aorta for the purpose of cardiopulmonary bypass or ventricular assist device, to E650, R701, R702, R703, R704 and Z743

When to Use

  • Use E682 when performing a graft on a major vessel (excluding the ascending aorta) specifically to facilitate cardiopulmonary bypass (CPB).
  • Use E682 when placing a graft on a major vessel to establish inflow or outflow for a ventricular assist device (VAD) implantation.
  • Use E682 as an add-on procedure when the primary surgical code is one of the mandated base codes: E650, R701, R702, R703, R704, or Z743.

Common Pitfalls

  • Billing E682 as a standalone procedure will result in automatic rejection; it must be submitted on the same claim as one of the six eligible base codes.
  • Attempting to use E682 for grafts involving the ascending aorta is incorrect, as the code explicitly excludes this anatomical location.
  • Failure to link the graft specifically to the CPB or VAD requirement can lead to audit recovery if the operative report does not clearly state the purpose of the vessel graft.

Billing Tips

  • Ensure the primary procedure (e.g., E650 or R701) is listed first on the claim to establish the surgical context required for the E682 add-on.
  • Apply age-based premiums (e.g., AGE_PREMIUM_UNDER_16Y) to the total procedural fee if the patient meets the specific age criteria, as E682 is a surgical procedure.
