This is an add-on fee payable in addition to `Z627 when renal calculi are disintegrated by any method during a percutaneous nephroscopy procedure. This fee is added to the surgeon's fee for Z627.

When to Use

  • Use E759 when performing active lithotripsy (e.g., laser, ultrasonic, or pneumatic) during a percutaneous nephrolithotomy (PCNL) billed under Z627.
  • Use this code when the primary objective of the percutaneous access is the fragmentation of a specific renal calculus that cannot be extracted intact.

Common Pitfalls

  • Billing E759 as a standalone procedure; it is strictly an add-on code and will be rejected if not submitted on the same claim as Z627.
  • Attempting to bill E759 for simple stone extraction or basket retrieval, which is already included in the base fee for Z627.
  • Confusing E759 with E772, which is specific to ureteroscopic lithotripsy; E759 is exclusively for percutaneous procedures.

Billing Tips

  • Ensure the operative report explicitly details the method of disintegration used to justify the add-on fee in the event of an audit.
  • Apply relevant age premiums or after-hours premiums (E409/E410) to the total procedural value if the criteria for those premiums are met.
