E846 – Rigid bronchoscopy rendered immediately after flexible bronchoscopy

This is an add-on payment to `Z327` (Flexible or rigid bronchoscopy) when a rigid bronchoscopy is performed immediately after a flexible bronchoscopy. As per the note on page , E846 is only eligible for payment when it is rendered for the treatment of a condition identified by the preceding flexible bronchoscopy.
When to Use
- Use when a flexible bronchoscopy (Z327) reveals a foreign body requiring a rigid bronchoscope for safe extraction.
- Use when a flexible bronchoscopy (Z327) identifies a central airway obstruction requiring immediate rigid dilation or stenting that cannot be performed via the flexible scope alone.
Common Pitfalls
- Claiming E846 without a corresponding Z327 on the same claim will result in automatic rejection.
- Attempting to claim E846 for routine rigid bronchoscopy performed as a primary procedure; it is strictly an add-on for treatment of findings identified during the preceding flexible scope.
- Billing E846 for bronchoscopies performed during the same anesthetic as a major lung resection, which is explicitly prohibited by the Schedule of Benefits.
Billing Tips
- Ensure your operative report clearly documents the findings from the flexible bronchoscopy that necessitated the immediate transition to the rigid bronchoscope to satisfy the P9 requirement.
- Apply age premiums (e.g., AGE_PREMIUM_LT16Y) to the E846 fee if the patient meets the specific age criteria, as these are calculated based on the procedural fee.
