When to Use

  • Apply this premium when injecting a sclerosing agent during a diagnostic or therapeutic nerve block (G235 or G238) to achieve permanent nerve ablation.

  • Claiming E958 without documenting the specific sclerosing agent or alcohol concentration used in the procedure note, which is required for audit verification.
  • Applying E958 to nerve blocks not explicitly listed in the schedule, such as standard local anesthetic blocks, which will result in a rejection.
