F012 – Fracture of hand or wrist - no reduction, rigid immobilization

Treatment of a Bennett's fracture (fracture of the base of the first metacarpal bone) without performing a reduction. The treatment consists of applying rigid immobilization, such as a cast. This service is a non-operative management option. See E584 for application of a plaster cast outside a hospital setting. Alternative treatments include closed reduction (F013) and open reduction (F015).
When to Use
- Use F012 when managing a stable Bennett's fracture that does not require manipulation or reduction and is treated solely with rigid immobilization.
- Use F012 for non-displaced hand or wrist fractures where the clinical decision is to splint or cast without attempting to realign the bone fragments.
Common Pitfalls
- Billing F012 when a closed reduction was actually performed; if any manipulation occurred to align the fracture, F013 is the required code.
- Attempting to bill F012 in conjunction with a consultation or assessment code on the same day without clear documentation of a separate, identifiable clinical reason for the visit.
- Incorrectly applying E584 for cast application when the procedure is performed within a hospital setting, as E584 is strictly for non-hospital environments.
Billing Tips
- Ensure your operative note explicitly states 'no reduction performed' to justify the use of F012 over the higher-valued F013.
- If the patient is under 16, remember that the age-based premium is calculated automatically on the base fee of F012, but ensure the patient's date of birth is correctly captured in the claim.
