This is a laboratory service for FSH (pituitary gonadotrophins). According to the preamble, it is an insured service only when performed by a physician in their own office for the physician's own patient. As a 'Reproductive medicine' test, it is only payable to physicians where point of care testing is necessary for their practice.
When to Use
- Use G015 when performing immediate point-of-care FSH testing in your office to assess menopausal status or pituitary function in a clinical setting where immediate results are required for patient management.
- Use this code when you are the treating physician and the test is essential for an immediate clinical decision, rather than referring the specimen to a community laboratory.
Common Pitfalls
- Billing G015 for tests related to infertility, IVF, or artificial insemination is strictly prohibited under Regulation 552 and will result in automatic rejection or audit recovery.
- Submitting G015 when the specimen is sent to an external laboratory is incorrect; those services must be billed by the licensed laboratory, not the physician.

Tests listed under “Reproductive medicine” and “Point of care drug testing” are only payable to those physicians where point of care testing is necessary for their practice.
