G125Caudal/lumbar epidural with catheter

OHIP Diagnostic & Therapeutic Procedures Code — DIAGNOSTIC AND THERAPEUTIC PROCEDURES · Schedule of Benefits

Caudal/lumbar epidural with catheter is a neuraxial nerve block for acute pain management. This service is classified as a Major Invasive Procedure and may be eligible for after-hours procedure premiums when rendered on a non-elective basis (). This procedure involves the placement of a catheter into the epidural space via the caudal or lumbar approach, allowing for continuous infusion or intermittent boluses of analgesic medication. As with all insured services, this procedure includes the common elements outlined in the Schedule of Benefits (see , ).

When to Use

  • Use G125 for the placement of a continuous epidural catheter for acute post-operative pain management or trauma-related analgesia.
  • Use G125 when performing a caudal epidural catheter placement for pediatric acute pain control.
  • Use G125 specifically when a catheter is left in situ for ongoing infusion, distinguishing it from single-shot blocks like G118.

Common Pitfalls

  • Billing G125 in conjunction with your own surgical procedure is prohibited under GP101, as the nerve block is considered part of the surgical service.
  • Attempting to bill Z432C for anesthesia support is a common rejection; you must use E030C or E031C when a separate physician provides sedation for this procedure.
  • Failing to apply the correct after-hours premium (E409-E413) for non-elective procedures performed outside standard hours, as G125 is explicitly listed as a qualifying Major Invasive Procedure.

Billing Tips

  • Ensure the procedure is clearly documented as non-elective to justify the use of after-hours premiums E409 or E410 for non-ED physicians.
  • When an anesthesiologist assists, ensure they bill E030C or E031C rather than standard anesthesia units, as extra units under GP97 are not payable for these support codes.
