G138 – Evoked potentials - professional component

Professional component for an evoked potentials test performed on the upper or lower limbs. This service represents the physician's work in interpreting the results of the test. The technical component is billed separately as G140. When only one limb is tested, this service must be claimed at 50% of the listed fee.

The physician's interpretation and report must be documented in the patient's medical record.
The physician must maintain documentation that describes the process by which the physician monitors quality assurance in accordance with professional standards.
