This service provides for the intralesional infiltration of 3 or more lesions. As indicated by the '+' symbol in the Schedule of Benefits, this is an add-on procedure and must be billed with an accompanying assessment or visit. Note: Intralesional injection of acne lesions with corticosteroids is not an insured service.

When to Use

  • Use G377 when treating 3 or more distinct keloids or hypertrophic scars with intralesional corticosteroids during a single patient encounter.
  • Use G377 for the therapeutic infiltration of 3 or more inflammatory lesions such as alopecia areata patches or granuloma annulare when medically necessary.

Common Pitfalls

  • Billing G377 for acne treatment will result in automatic rejection or clawback, as intralesional corticosteroid injections for acne are explicitly excluded from OHIP coverage.
  • Attempting to bill G377 alongside counselling codes like K013 or K033 will trigger a rejection because G377 is not on the list of permitted exceptions for concurrent billing.
  • Billing G377 without an accompanying assessment or visit code (e.g., A007) will cause the claim to fail the add-on procedure validation rule.
