G375 is payable for the intralesional infiltration of one or two lesions. The '+' symbol beside the code on the Schedule indicates it is an add-on procedure, meant to be billed in conjunction with an appropriate assessment. Note: Intralesional injection of acne lesions with corticosteroids is not an insured service. For infiltration of three or more lesions, see G377.
When to Use
- Use G375 for the intralesional injection of keloids or hypertrophic scars when treating one or two distinct lesions.
- Use G375 for the therapeutic infiltration of localized inflammatory dermatoses, such as alopecia areata or granuloma annulare, limited to two sites.
Common Pitfalls
- Billing G375 for acne treatment is a common audit trigger and will result in a rejection, as this is explicitly excluded from OHIP coverage.
- Submitting G375 for three or more lesions is a billing error; you must use G377 for three or more sites to avoid payment discrepancies.
- Attempting to bill G375 as a standalone service without an accompanying assessment code will lead to rejection, as it is an add-on procedure.
