G399 – Transvaginal sonohysterography

This service covers the introduction of a catheter for transvaginal sonohysterography. It includes the procedure of catheter insertion and may also involve the injection of contrast media. Payment for G399 is contingent upon the performance of the full transvaginal sonohysterography service, billed as either J165 or J476 by the same or another physician.
When to Use
- Use G399 when you perform the catheterization and saline infusion required to facilitate a transvaginal sonohysterography procedure billed as J165 or J476.
- Bill G399 when acting as the proceduralist inserting the catheter for a sonohysterogram, even if the ultrasound interpretation (J165 or J476) is performed by a radiologist.
Common Pitfalls
- Billing G399 without a corresponding J165 or J476 claim in the system will result in an automatic rejection as the payment is strictly conditional.
- Attempting to bill G399 for a standard transvaginal ultrasound (J165) that does not involve saline infusion or catheterization will lead to audit recovery.
Billing Tips
- Ensure the J165 or J476 claim is submitted for the same date of service to satisfy the OHIP payment dependency requirement.
