G436 – Visual fields - kinetic (professional component)
OHIP Diagnostic & Therapeutic Procedures Code — DIAGNOSTIC AND THERAPEUTIC PROCEDURES · Schedule of Benefits
This fee code represents the professional component (interpretation and report) for a kinetic visual fields test. A permanent record of the test is a required element of the service. As a professional component, it must be rendered personally by the physician. It is billed in conjunction with its corresponding technical component, G857.
When to Use
- Use G436 when you have personally interpreted a kinetic visual field test, such as Goldmann perimetry, for a patient requiring assessment of peripheral vision loss.
- Bill this code specifically when you are providing the professional interpretation for a test performed in an office setting, ensuring the technical component G857 is billed alongside it for the same date of service.
Common Pitfalls
- Billing G436 without the corresponding technical component G857 will result in a rejection, as the professional component requires the technical fee to be processed in the same claim.
- Attempting to bill G436 for automated static perimetry (e.g., Humphrey Field Analyzer) is incorrect, as G436 is strictly for kinetic visual field testing.
Billing Tips
- While the technical component G857 is restricted for hospital in-patients, you may still bill G436 for your professional interpretation if you are a consultant providing the report for an in-patient.
