G857 – Visual fields - kinetic (technical component)
OHIP Diagnostic & Therapeutic Procedures Code — DIAGNOSTIC AND THERAPEUTIC PROCEDURES · Schedule of Benefits
G857 represents the technical component for performing a kinetic visual field test, which includes the creation of a permanent record. The technical component fee covers the use of the necessary equipment, supplies, and personnel to conduct the test. This service is typically performed in conjunction with the professional component, G436, which covers the physician's interpretation of the results. The service must adhere to the requirements for technical components outlined in of the Schedule of Benefits, including quality assurance monitoring.
When to Use
- Use G857 when performing kinetic perimetry (e.g., Goldmann visual fields) to document the technical execution of the test in your office.
- Always pair G857 with G436 when you are billing for both the technical performance of the kinetic field and the subsequent professional interpretation.
Common Pitfalls
- Billing G857 without maintaining a permanent, retrievable record of the visual field plot, which is a mandatory requirement for the technical component.
- Failing to account for the 13.9% reduction in payment when G857 is performed within a hospital setting, as mandated by the Schedule of Benefits.
- Billing G857 for automated static perimetry, which is technically incorrect as G857 is specifically designated for kinetic visual field testing.
Billing Tips
- Ensure your office maintains a formal quality assurance log for the equipment used to perform G857 to satisfy the documentation requirements for technical components.
