Chemodenervation injection of an additional individual peripheral motor nerve and/or its branches using phenol, ethyl alcohol or a similar non-anaesthetic chemical agent for the purpose of reducing focal spasticity. This code is intended to be used in addition to G485 for each major nerve treated after the first on the same day. The procedure may include electromyography (EMG) guidance for the injection(s).

When to Use

  • Use G486 for the second and subsequent major peripheral motor nerves treated with phenol or alcohol during the same session as the primary G485 injection.
  • Apply this code when performing multi-level chemodenervation for focal spasticity where multiple distinct nerve branches require chemical neurolysis.

Common Pitfalls

  • Billing G486 for botulinum toxin injections into the same muscle will result in a nil payment; use specific botulinum toxin codes instead.
  • Submitting G486 without a corresponding G485 on the same claim will trigger a rejection as it is strictly an add-on code.
  • Using G486 for injections involving local anaesthetic agents is incorrect; these must be billed under the Nerve Block section.

  • Ensure your documentation clearly identifies each distinct nerve branch treated to justify the multiple units of G486 claimed alongside the primary G485.
