G814 – Orthoptic examination

Orthoptic examination must include quantitative measurement of all cardinal positions of gaze (straight ahead, left, right, up, down, tilt right and tilt left), sensory testing for binocular vision suppression, cyclodeviation, retinal correspondence and interpretation. Orthoptic examination is eligible for payment in addition to an ophthalmology consultation or visit. The examination must be rendered by an orthoptist who is certified by the Canadian Orthoptic Council and employed by the ophthalmologist or a public hospital. The interpretation component of the examination must be personally rendered by the ophthalmologist.
When to Use
- Use G814 when a Canadian Orthoptic Council certified orthoptist performs the full battery of measurements, including cardinal gaze and sensory testing, while you provide the final interpretation.
- Use this code to bill for orthoptic services performed by your employed staff during the same encounter as your ophthalmology consultation or assessment.
Common Pitfalls
- Billing G814 when you personally perform the examination instead of your certified orthoptist; in this case, you must bill A230 instead.
- Submitting G814 without explicit documentation of all required measurements, including cyclodeviation and retinal correspondence, which are mandatory for audit compliance.
- Claiming G814 when the orthoptist is not directly employed by your practice or the public hospital where the service occurs.
Billing Tips
- Ensure your clinical notes clearly delineate the orthoptist's measurements from your own interpretation to satisfy the requirement that the ophthalmologist personally renders the interpretation component.
