G914 is for a percutaneous diagnostic lumbar facet medial branch block using ultrasound guidance for the initial site. Payment for this service is conditional on a prior fluoroscopically guided facet injection for the same site(s) within the last 12 months by the same physician. Commentary: Ultrasound images must be of sufficient quality to clearly identify the injection site and needle placement at the junction of the transverse process and superior articular process. ()
When to Use
- Use G914 when performing a subsequent diagnostic medial branch block on a patient who has already received a fluoroscopically guided block at the same site from you within the previous 12 months.
- Use G914 when transitioning a patient from fluoroscopic guidance to ultrasound guidance for repeat diagnostic blocks to minimize radiation exposure.
Common Pitfalls
- Billing G914 without a prior fluoroscopically guided procedure (G910/G911) by the same physician within the last 12 months will result in an automatic rejection.
- Failure to store or attach the required ultrasound images demonstrating the needle at the junction of the transverse process and superior articular process will lead to clawbacks during an audit.

- Always pair G914 with G915 when treating multiple levels to maximize the daily limit of 7 sites per patient.
- Ensure your clinical notes explicitly reference the date of the previous fluoroscopically guided procedure to support the eligibility criteria for G914.
