G914 is for a percutaneous diagnostic lumbar facet medial branch block using ultrasound guidance for the initial site. Payment for this service is conditional on a prior fluoroscopically guided facet injection for the same site(s) within the last 12 months by the same physician.
Commentary: Ultrasound images must be of sufficient quality to clearly identify the injection site and needle placement at the junction of the transverse process and superior articular process. ()
