K036 provides payment for the completion of the northern health travel grant application form. As per the commentary, this fee is payable to both the referring physician and the specialist physician for the same patient.
An insured service listed in the Schedule that includes the completion of a form is not eligible for payment to a Supervising Physician if rendered by a Medical Trainee unless the form has been reviewed and signed by the Supervising Physician.
